Becoming A HealthTrust Vendor

Thank you for your interest in becoming a HealthTrust contracted vendor. Before you complete and submit the Prospective Vendor Questionnaire, please review the following:

• Submitting a completed questionnaire does not ensure that your company will become an HealthTrust contracted vendor.
• Please review the current HealthTrust Contract Schedule to see if the current contract for your company's product or service is scheduled to expire this year.
• Your company will only be contacted if the HealthTrust contract for your product category is up for renewal or bid. If your product category is scheduled to remain on contract, your submission will be kept on file for future reference. When and/or if your product category is up for renewal or bid, HealthTrust will contact you for further information.
• Only companies that have the ability to supply all of HealthTrust's membership can be considered as a contracted vendor. Please view the required criteria before submitting your request.
